Step 1) Demande de don

Step 2) Demande examiné par BLUME Haiti

Step 3) Soumèt Rapò de don

Si Blume Ayiti apwouve demann ou pou sipò, apre ou resevwa don an ou dwe ranpli fòm Rapò de don. Tanpri swiv lyen ki anba a ranpli fòm nan epi reponn tout kesyon nèt. Apre ou fini tanpri klike soumèt, epi voye e -mail ak nenpòt materyèl siplemantè (tankou foto, oswa videyo kote wap itilize don de BLUME Haiti ) nan info@blumehaiti.org. 

Fòm nan dwe retounen nan Blume Ayiti pa plis pase yon mwa apre se don nan te resevwa.

If you are a member of a Haitian Music School and you would like to request support in the form of funding, instruments, supplies, teacher salaries, teacher training, scholarships, etc., please carefully read and follow the instructions below.

Step 1) Request Support

Please follow the links below to fill out the English version of the Donation Request Form and answer all questions completely. Upon completion please click submit. Please e-mail any additional materials related to your request to info@blumehaiti.org. 

For the moment, BLUME Haiti is only able to take requests for less than $500 US (your request should be for no more than $500 US.) 
Please make sure your request is realistic and specific, and justify your need for support. 
For example,  if you request $300 for teacher salaries, include specific dollar amounts and explain for how many teachers, for how many hours a week, and for how many months the salaries will be used.

​Step 2) Request Reviewed by BLUME Haiti

The BLUME Haiti Programming and Disbursement Committee  will review your application and decide whether to fund (partially or completely) your request. We will notify you of our decision. We ask for your patience as this committee typically meets every other month.

​Step 3) Submit Donation Report

If BLUME Haiti approves your request for support, upon receipt of the donation the beneficiary must complete the form below. Please fill out either the French or English version of the form and answer all questions completely. You do not need to fill out both versions. Upon completion please click submit, and e-mail any supplementary materials (such as photos, or video of BLUME Haiti'd donation being used) to info@blumehaiti.org.

The form must be returned to BLUME Haiti by no more than one month after the donation is received.
